Information you should bring on your first visit:
- If you have already had tests (such as X-rays or MRI's) regarding your current problem, please bring in the films or reports if you
have them.
- If you have insurance, please bring in your insurance
card.
- If your insurance requires a referral, please obtain
one.
Forms
- Please download, print and fill out the Patient History form
below before your first visit.
- If you have been in an auto accident, please fill
out the Patient History form and the Auto
Accident
History form. Also bring in a copy of your
accident report if you have one.
- If you have been injured at work, please download
the Patient History form and the Workman's Comp Authorization form.
For all NJ work comp injuries, it is necessary for your employer fill out the
Workman's Comp Authorization form before your first visit. We are unable to
treat you until this form is completed and signed.
If you are unable to print these forms, we will make time
for you to fill them out in our office.

What to expect on your first visit
| Step 1 |

A Detailed History |
| A detailed history gives the Dr. Koch a
background about your health, such as surgeries, accidents,
the onset of your condition, and other details affecting
your current health. |

| Step 2 |
 |
An Examination |
| A chiropractic, orthopedic and neurologic
exam will help us to find the exact cause and location of
your condition. |

| Step 3 |
 |
X-ray |
| If Dr. Koch believes your case warrants
X-rays, films can be taken in our on-site x-ray suite. |

| Step 4 |
 | The
Adjustment |
| If Dr. Koch finds that you do have Vertebral
Subluxations, a gentle adjustment is given in order to
relieve the nerve irritation and begin the healing process. |
